We have an exciting opportunity for a Community Staff Nurse to join our Colliery District Nursing team on a part-time basis. We are an innovative service covering GP practices in the Cannock locality and we are seeking dedicated, compassionate and highly motivated nurses.
As a Community Staff Nurse, you will deliver a high-quality service, working closely with GPs and multi-disciplinary teams in addressing a range of care issues across the spectrum of health conditions, and preventing avoidable hospital admissions.
You will work independently in a community setting, participating in the ongoing assessment, planning, implementation, and evaluation of programmes of care, adhering to policies and procedures.
You must be a qualified nursing practitioner with current NMC registration. Previous experience of working in the community is not an essential requirement; however, you do need to demonstrate a commitment to delivering high-quality targeted personalised care.
You will be required to work flexibly on a rota system across a 7-day working week.
Main Duties of the Job
You will be part of the District Nursing service providing healthcare and will work in a variety of settings. This is a dynamic post that requires the post holder to work closely with uni/multidisciplinary teams/agencies in addressing a range of care/support issues across the spectrum of conditions, preventing unnecessary hospital admissions and improving the health and wellbeing of the population served. The nature of the service will be determined by local service specifications.
Duties include:
1. Assessing new service user referrals and care/support needs.
2. Formulating, developing, implementing, and evaluating programmes of treatment/care/support for individuals, families, and carers.
3. Promoting independence/self-management and maintaining safety.
4. Increasing functional ability.
5. Improving health and wellbeing including mental health.
6. Interventions in disease/condition management.
7. Preventing and reducing health inequalities.
8. Palliative and end of life care.
9. Undertaking Tissue Viability Risk Assessments.
10. Forming a professional judgement of an individual's health condition.
11. Prioritising treatment and care for individuals according to their health status and needs.
12. Bladder, catheter, and bowel care.

Person Specification
Qualifications
* Degree in Nursing or equivalent.
* Registration with NMC.
Experience
* Relevant experience to the role.
* Experience of working effectively across different agencies and with other disciplines.
* Experience of providing clinical information and advice to others.
* Supporting service users, families, and carers in dealing with the delivery of healthcare in the community.
Driving Licence
* Full driving licence and access to a car.
