Job summary The Memory Service offers an exciting opportunity to work with older adults who are experiencing memory difficulties. The post holder will work closely with the service user and their supporters/next of kin/carers to ensure that their experience from assessment, through to diagnosis and post diagnostic planning, ensuring that their care is personalised with them at the centre. Coordinate and actively participate in the daily running of the Case Management Team.
