Job Summary
CDDFT Community Respiratory Nursing Service is pleased to advertise a full-time, permanent administrator position based at Chester-Le-Street, to join the team supporting the nursing service across County Durham and Darlington.
This post is based at Chester-Le-Street Hospital.
This post offers flexible, Monday-Friday working hours between 8:30am and 5:30pm, which can be negotiated at interview.

Main Duties of the Job
To work with staff across the Community Respiratory Nursing service in a non-patient facing clinical role:
1. Be responsible for accepting and completing electronic tasks with the help of nursing staff as required.
2. Allocate work appropriately using electronic systems.
3. Identify and apply appropriate care plans, setting frequency and dates.
4. Carry out administrative and housekeeping tasks to support the service and patient care.
5. Participate and provide training around the electronic patient management system and the functioning of the team in their area.
6. Provide and receive routine information requiring tact or persuasive skills; provide and receive complex or sensitive information; provide advice, instruction, or training to the team.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a center for planned care in Bishop Auckland and provide care from community hospitals in Chester-Le-Street, Shotley Bridge, Barnard Castle, Sedgefield, and Weardale, as well as over 80 other community-based settings and providing care in patients' homes.

Job Description
Job Responsibilities
1. Manage electronic referrals and tasks, taking appropriate action while seeking advice if required.
2. Manage own workload with an understanding of priorities to assist the team’s function.
3. Provide staff with advice regarding administrative procedures and access as required.
4. Coordinate clinical work for colleagues within SystmOne, ensuring allocated visits are geographically close while maintaining continuity of care.
5. Communicate effectively with patients, relatives, and healthcare practitioners, even in challenging situations.
6. Demonstrate effective time management skills and participate in monitoring and reviewing team performance.
7. Gather key audit data from electronic systems and produce reports to Clinical Lead/Matron as requested.
8. Promote an environment where health promotion and informed patient choice are encouraged.
9. Undertake relevant training to enable high-quality service provision.
10. Complete effective record keeping in accordance with Trust policy and legal requirements.
11. Commit to continuous personal development/lifelong learning by attending mandatory training and relevant courses.
12. Participate in supervision/1:1 sessions and contribute to objective setting and personal development planning.
13. Contribute to all activities associated with the team's response to current trends and changes within the NHS.
Communications and Working Relationships
The post holder will communicate in various ways to allocate work and discuss patients with Single Point of Access staff, primary care staff, Allied Health and Social Care professionals, and hospital staff, patients, and relatives.
They will need to form professional relationships with all parties to communicate effectively while respecting their views, autonomy, and culture.
To effectively liaise with patients, carers, and a range of Health and Social Care staff to facilitate treatment/rehabilitation plans.
To report effectively to the team on performance/progress.
Person Specification
Experience
Essential:
1. Evidence of team working.
2. Experience of using trust electronic systems such as SystmOne and telephone call handling.
Desirable:
1. Understanding of community nursing and systems of working.
Special Requirements
Essential:
1. Able to travel across Trust sites to meet the requirements of the post.
Special Skills and Knowledge
Essential:
1. Good verbal and written communication skills.
2. Good organisational and facilitation skills.
3. Ability to work independently.
4. General IT skills.
Desirable:
1. Using data extraction.
2. Preparing Excel spreadsheets.
3. Clinical knowledge.

Qualifications
Essential:
1. NVQ level 3 or equivalent qualification.
