Main duties of the job
The role will involve offering consultations to patients within a practice and supporting the needs of the patients as required.
* Clinical Assessments - the successful candidate must be able to offer clinical assessment for a range of presentations within a standard clinic.
* Treatment Decisions - the successful candidate should be able to independently identify appropriate treatment and where required order necessary investigations.
* Prescribing Medication - as part of a consultation the need to prescribe medication may be needed. The successful candidate should have an appropriate qualification in prescribing and show knowledge of the latest prescribing guidelines.
* Referrals - where appropriate, referrals to secondary care may be required as part of the process appropriate forms may need to be completed prior to referral, knowledge of local guidelines is desirable
* Pathology requests - process and interpret pathology and other test results as required.
* Support the team in dealing with clinical emergencies
* Recognise, assess and refer patients presenting with mental health needs
Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health

* Work within own competence and professional code of conduct to provide consultations to patients in a clinical environment, both face to face and via the telephone where required.
* Produce accurate, complete records of patient consultations, consistent with legislation, policies and procedures.
* Deliver care according to NSF and NICE guidance
* Assess effectiveness of care delivery through self and peer review, bench marking and formal evaluation
* Work within the wider team, supporting colleagues in the support of meeting the needs of our patients
* Participate in the maintenance of quality governance systems and processes across the organisation and its activities
* Utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required
* Support and participate in shared learning across the practice and wider organisation
* Participate in the management, review and identify learning from patient complaints, clinical incidents and near-miss events utilising a structured framework (eg root-cause analysis)
* Understand and apply legal policy that supports the identification of vulnerable and abused children and adults, being aware of statutory child/vulnerable adult health procedure and local guidance
* Work within policies relating to domestic violence, vulnerable adults, substance abuse and addictive behavior, and refer as appropriate

* Post graduate MSc or equivalent - Advanced Practice Qualification
* Qualified Nurse Prescriber
* Registered Nurse
Experience
* NHS experience
* Experience of working within a nursing team
* Experience of dealing with vulnerable patients
* Knowledge of needs of patients with a long term condition
* Knowledge of Minor illnesses and appropriate treatment
* Ability to discuss difficult and possibly communicate difficult messages to patients/their family
* Ability to identify the needs of patients with long term conditions
* Knowledge of public health issues in the local area
* Awareness of local and national health policy
* Knowledge of PGD/PSD and associated policies
* Experience of EMIS Web
* Able to offer additional services to patients outside of minor illness clinics (cervical cytology, vaccinations, QOF reviews)
