You will prepare and manage complex multidisciplinary assessments and ongoing case management for individuals with unstable and unpredictable conditions, with guidance from senior team members. Responsibilities include creating tailored support plans, participating in brokering and tendering processes, and offering assessments to carers for their distinct needs. The aim is to sustain individuals within the community, particularly those with challenging behaviors. This role requires working closely with the team manager and other senior care managers.
