At Bluebell Wood Children's Hospice, we care for children and young adults with life-shortening and life-threatening conditions. We support around 300 families both in our hospice and in family homes.
Our support services are wide-ranging and bespoke to each family, including end-of-life treatment and care, in-house and community short breaks, counselling, sibling support groups, music therapy, home visits, and much more. It costs more than £6.5m to keep our doors open for families across South Yorkshire and beyond. We receive around 16% of our funding from government sources.
Main duties of the job
The role
We are seeking qualified and experienced nurses to join our rapidly expanding Community Team. Working as part of our clinical team, you will provide and ensure a high standard of holistic care to children with life-limiting/life-threatening conditions, including opportunities for children to have flexibility and choice of care within the home, hospice, or other community settings. You will deliver child-focused, family-centred palliative, end-of-life care and care after death, alongside members of the interdisciplinary team. As part of our hospice at home service, you will work predominantly in the community and therefore must be willing and able to embrace a mobile and agile working model. However, there is an expectation of working across all areas including hospice and community/hospital settings.

To fulfil the role effectively, you will be NMC registered and have relevant clinical experience, and be able to work flexibly and collaboratively across the care services.

About the job
Main purpose of the job: As a Bluebell Wood staff nurse, you will:
1. Work as a skilled member of the Care Services Team, providing high quality nursing care to meet the diverse and individual needs of babies, children, young people and their families.
2. Deliver child-focused, family centred palliative and end-of-life care in the community.
3. Support colleagues, students and trainees whilst on duty.
Clinical
1. Assess, plan, evaluate and deliver high quality nursing care to meet the diverse and individual needs of children, young people and their families.
2. Caring for children with a wide range of complex clinical needs and technology dependence (for example ventilation, tracheostomies and enteral feeding devices).
3. Work with other members of the multidisciplinary care team to ensure that plans of care are carried out to the agreed standard.
4. To ensure an ongoing welcoming, caring and safe environment is provided for the children, young people and their families.
5. Identify and escalate significant changes in a child or young person's condition or well-being, outside of your area of competence.
6. Establish therapeutic relationships with children, young people and their families.
7. Deliver care after the death of a child, supporting their family at home and in the hospice.
8. Act as an advocate for the child, young person and family, providing them with information and support and directing them to the relevant support services where appropriate.
9. Communicate complex and sensitive information effectively to children, young people and their families, and other staff, overcoming any barriers to understanding and providing support during distressing or emotional events.
10. Develop and maintain clinical skills and knowledge necessary to provide holistic, evidence-based nursing care. This includes completion of the hospice clinical skills competencies and expanded role responsibilities, following appropriate training.
11. Assist with ongoing education and training of colleagues, students and trainee practitioners, as required.
12. Undertake initial health needs assessment of new referrals and present them to the referral and assessment panel.
13. To work closely with the Transition Coordinator to identify the transition needs of young adults and their individual bespoke support needs.
14. Support the development of advance care plans.
Leadership
1. Contribute towards quality and improvement initiatives being undertaken, as directed by the Care Team Leaders.
2. Report near misses and untoward incidents, complaints, clinical emergencies, injury or medicine administration errors, as outlined in the hospice policies and procedures.
3. Lead shifts, in the absence of a Community Team Leader.
Education:
1. Actively assist in the education and practical training of colleagues, students and trainees.
2. Supervise and orientate learners and new staff.
3. Participate in surveys, audit, research and facilitating clinical trials as required.
4. Assess students on placement, in line with the requirements of the Nursing and Midwifery Council.
Governance

3. Successfully complete required competences as defined by Bluebell Wood.
4. To maintain confidentiality surrounding the child or young person's admission and treatment at all times.
