Job overview
We are focused on medical patients' safety experience throughout the admission, transfer, and discharge. We are currently looking for a Band 5 Registered Nurse to join our team, and the successful candidate will progress their nursing skills in nursing assessment, admission, treatment of various medical pathways, transfer, and discharge process.
Main duties of the job
1. Communicate effectively with patients and their relatives, ensuring accurate and seamless information is conveyed and documented.
2. Ensure that accurate and legible patient records are kept in line with professional and legal requirements.
3. Act as the identified nurse for critically ill patients by assessing, planning, implementing, and evaluating their care.
4. Participate in taking an active role in planning effective, timely, and safe discharges.
5. Work to eliminate all avoidable infections by acting as a role model in infection prevention and control to all staff, visitors, and patients.
6. Provide training and support to new and junior staff in accordance with Trust policies and procedures.
7. Identify and minimize clinical and non-clinical risk to minimize risk of harm to patients, staff, and visitors.
8. Involve patients in their care and ensure their understanding of treatment plan and care options before gaining informed consent for care delivery and interventions.
9. Ensure that Privacy and Dignity of patients is maintained at all times and work to minimize mixing of male and female patients and sharing of bathroom/toilet facilities.
10. Participate in department meetings when required.
11. Ensure compliance with professional expectations as outlined in the NMC Code of Professional Conduct.
12. Participate in an annual appraisal with relevant reviews and to follow your personal development plan as agreed with your line manager.

* Recent clinical experience in an acute Trust.
* NMC Registration.
* Delivery of evidence-based practice.

* Mentor Qualification or ENB998 Completion of Preceptorship.
* Evidence of facilitating learning in practice.
* Evidence of ongoing personal and professional development.
Knowledge
