NHS Devon are seeking to recruit a range of CHC Assessor posts across Devon.
The post holder will be part of the Nursing and Quality Directorate for NHS Devon Integrated Care Board (ICB) their aim will be to improve the quality of care and value for money of services commissioned for individuals who are the responsibility of NHS Devon ICB.
The post holder will play a key role in supporting the Continuing Healthcare (CHC) service across Devon. Working within the remit of the current National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care.
The post holder will be responsible for assisting in the delivery of person-centred assessments in order to identify any primary health care need which may result in eligibility for CHC (or associated funding stream), ensuring the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care is implemented and complied with according to the principles and processes.

Main duties of the job
Work within a dynamic team, delivering an effective service as part of the NHS Continuing Healthcare team. The post holder must have basic knowledge and experience of working within the health sector or Social Care sector.
* Highly motivated and enthusiastic, will provide and coordinate administrative services to include sourcing and collating records, populating national tools for CHC, the preparation of agendas and minutes, taking appropriate follow-up action as required.
* Liaison with other professionals, in person or virtually to identify evidence of health care needs, using various computer systems appropriately.
* Ability to read and understand clinical records and notes, demonstrating ability to collate a full set of documents and evidence to support the assessment of need.
* The post holder will work under the supervision of Clinicians who will use this evidence to form the basis of their CHC assessment and will assist with typing the final document as required.
* To use the information collated to support the CHC process and contribute to the clinical review process, to include confirming that a review had been completed and the outcome. To identify where there is a change in care needs and initiate a more in-depth review where required.
* The ability to maintain professional relationships and rapport with individuals, their families, carers and health and social care colleagues.

Person Specification
Qualifications
* Educated to Level 3 Diploma in Clinical Healthcare Support or equivalent level of qualification or significant equivalent previous proven experience.
* Willingness to work towards the completion of a Level 5 Continuing Healthcare Associate Practitioner Apprenticeship or possess equivalent knowledge and experience. desirable
Experience
* Experience of working in a health or social care environment
* Ability to transcribe and populate assessments reports of a high written standard
* Good knowledge of NHS continuing healthcare legislation, the National Framework and associated guidelines
Skills & Attributes
* Administrative or relevant experience including initiating and maintaining office systems, including a working knowledge of Microsoft Office including Word and Excel
* Time management skills with the ability to re-prioritise
Communications
* Ability to work effectively as part of a team
* An ability to maintain confidentiality and trust in a professional, calm, and efficient manner.
* Positive and flexible approach to work.
* Ability to remain calm when dealing with people who have an emotional and/or financial interest in CHC decisions, demonstrating tact, diplomacy, and empathy

* Full driving license
* Ability and access to transport to enable travel throughout the Devon footprint. Although generally the postholder will be aligned to a specific locality and work as part of a team
* Ability to use a keyboard for lengthy periods, with a high degree of accuracy
