Job Title: Qualified Social Worker (QSW) - Adult Care Management

Hours: Full-time
Are you a compassionate and experienced Social Worker looking to make a meaningful impact in adult care management? Liverpool City Council is seeking a Qualified Social Worker to join our Adult Care Management team. In this role, you'll support adults with varying needs, helping them to access services, achieve greater independence, and improve their quality of life.
Key Responsibilities:
1. Case Management: Oversee a caseload of adults with diverse support needs, conducting assessments, developing care plans, and providing ongoing support.
2. Advocacy: Act as an advocate for adults, ensuring they are empowered to make informed decisions about their care and support options.
3. Collaboration: Work closely with multi-disciplinary teams, family members, and external agencies to provide comprehensive care management.
4. Crisis Intervention: Respond to crisis situations, delivering appropriate interventions and connecting clients with necessary resources.
5. Legal & Statutory Compliance: Ensure all work aligns with statutory requirements and best practices in adult social care, including the Care Act and Mental Capacity Act.
Requirements:
1. Qualified Social Worker: Must hold a recognized Social Work qualification and be registered with Social Work England.
2. Experience in Adult Social Care: Demonstrated experience in care management for adults, with an understanding of the specific challenges and needs of adult clients.
3. Strong Communication Skills: Able to communicate effectively with clients, families, and multidisciplinary teams to create clear and supportive care plans.
4. Knowledge of Statutory Legislation: Familiarity with legislation relevant to adult social care, such as the Care Act, Mental Capacity Act, and safeguarding protocols.
5. Organizational Skills: Ability to manage caseloads effectively, with strong prioritization and time-management skills.
