Job title - Virtual Wards Onboarding Nurse Band 6 Directorate/ department - Integrated Care Division - Medicine & Integrated Care Division Location of work - Cross-site Hours - Full-time Reports to - Virtual Wards Business Manager Accountable to - Divisional Director/SRO Job purpose The post holders will rotate every three months alternately between the following two areas, spending six months in each: At the NWL Virtual Hospital, the Onboarding Nurses will: Provide high quality, effective, individualised care to all patients by assessing care needs and developing, implementing and evaluating them without supervision Assist the Digital lead Nurse in providing care in a digital remote context and deputise in their absence Monitor standards of care and take appropriate action to maintain and improve them when monitoring patients remotely. Respond to patients, relatives and carers concerns as they arise and take remedial action as required Assist hospital teams in achieving a digital personal health record, helping to ensure the team are able to provide timely review/transfer/discharge of patients.
