About the Role:
We are looking for an enthusiastic and dedicated Adult Social Worker to join the Swift Response Team, which provides immediate support to adults who need rapid social care interventions. The ideal candidate will have experience in managing urgent referrals, crisis intervention, and facilitating quick, person-centered care planning for individuals in need of immediate support. This role is critical in providing short-term care to those in need of swift social work services in response to emergencies or urgent situations.
Key Responsibilities:
* Respond to urgent referrals and assess adults facing an immediate crisis within the community, including those with complex needs, mental health concerns, or safeguarding issues.
* Carry out initial assessments in accordance with the Care Act 2014 and Mental Capacity Act 2005, focusing on urgent risk management, safeguarding, and immediate care planning.
* Provide swift interventions aimed at stabilising the individual’s situation and preventing the need for hospital admission or further escalation.
* Develop short-term care plans, identifying needs, resources, and appropriate support mechanisms to address the crisis.
* Ensure the promotion of independence and empowerment by supporting individuals in making informed decisions about their care options.
* Collaborate with multi-disciplinary teams (MDTs), including healthcare professionals, housing, and community services, to ensure comprehensive, integrated support.
* Identify and assess safeguarding concerns and take necessary actions to protect vulnerable adults in line with local and national safeguarding protocols.
* Ensure that all actions are taken in compliance with the Safeguarding Adults Board procedures and any other relevant local authority policies.
* Review and monitor the effectiveness of interventions, ensuring safety and suitability in the response to each individual’s needs.
* Liaise with healthcare providers, housing services, community support, and other agencies to ensure a coordinated and swift response to the needs of service users.
* Work effectively in a team environment, supporting colleagues and sharing information and resources to enhance service delivery.
* Maintain accurate and up-to-date case records using the appropriate IT systems and in compliance with data protection and confidentiality policies.
* Prepare reports for case reviews, safeguarding meetings, and other relevant forums.
* Ensure compliance with Care Act 2014, Mental Capacity Act 2005, and other relevant legislation in all casework.
Qualifications and Experience Required:
* Degree in Social Work (or equivalent) from a UK university.
* Registration with Social Work England.
* Post-qualifying experience in adult social care, hospital discharge, or community-based care planning.
* Strong knowledge of Care Act 2014, Mental Capacity Act 2005, and NHS Continuing Healthcare (CHC) frameworks.
* Ability to manage complex casework and risk assessments.
* Strong problem-solving and decision-making skills.
* Experience working in a multi-agency environment, ensuring a coordinated approach to care.
