As the Transformation Manager for LDAN, you will play a pivotal role in shaping and delivering a comprehensive strategy that spans all age groups; adults, older adults, and children and young people. Working closely with the Associate Director of Mental Health, Learning Disabilities, and Autism Commissioning, as well as a network of directors and senior managers, you will lead the transformation of care pathways, ensuring that services are responsive, inclusive, and accessible.
In this influential role, you will drive the delivery of transformation plans across the LDAN agenda, collaborating with system partners to deliver high-quality, integrated services. You will support the redesign of services, enhance commissioning efforts, and contribute to performance management strategies, ensuring that services are not only effective but also aligned with the needs of the Somerset population.

Job responsibilities
PURPOSE OF THE ROLE:
The Transformation Manager of Learning Disabilities, Autism and Neurodiversity (LDAN) will work closely with the Associate Director of Mental Health, Learning Disabilities and Autism Commissioning, directors and senior managers for their area (internally and externally) to ensure oversight and delivery of joint responsibilities. They will lead in the areas of:
1. Supporting the development and the delivery of the strategy for LDAN across all ages (adult, older adults and Children and Young People (CYP)), that in turn delivers the outcomes required for the people of Somerset when accessing care.
2. Lead on the delivery of the transformation agenda for pathways of care within LDAN.
This is a dynamic, impactful role with the opportunity to lead real change in the lives of individuals with learning disabilities, autism, and neurodiversity across Somerset.
Person Specification
Qualifications
* Educated to Masters level or equivalent level of experience working at a senior level in a specialist area.
* Evidence of post qualification and continuing professional development.
* Member of relevant professional body.
