The main purpose of the job is to provide clinical leadership for Integrated Community Nursing Teams and support wider community health and social care teams to ensure safe and effective quality care delivery. The key responsibilities include securing and assuring the highest standards of clinical care, providing clinical leadership and support for staff, service users, and their relatives.

• Registered General Nurse (RGN) qualification
• Community Specialist Practitioner (CSP) qualification
• Sound clinical and operational knowledge of community services

• Excellent teamwork and collaboration skills
• Ability to travel within the Trust footprint
