To provide specialist advice to support the assessment, planning and management of patients accessing the Breast Care Service.
Main duties of the job
* To provide clinical expertise within Breast Care Team, acting as a role model in the delivery of patient care.
* The post holder will work closely with the multi-disciplinary team locally and externally to ensure standards of care maintained and work within the Breast Care Team to promote service development within the speciality.
* Act as a key worker to provide information and support for patients accessing the Breast Care Service.
* The post holder will have the knowledge and skills to work autonomously managing his/ her caseload of patients.
* The post holder is expected to act in a manner consistent with the NMC Code of Professional Conduct, carrying out his/her duties in accordance with Trust policies and procedures.

Job responsibilities
* Ensure patient safety is maintained.
* To undertake assessment, planning and evaluation of breast care patients.
* To promote a culture of demonstrable best practice and integrated evidence based practice.
* To provide specialist clinical advice, support and education to professions/ staff and patients.
* Act as patient advocate where relevant, facilitating informed discussion concerning treatment options and ensuring informed consent.
* Work collaboratively and co-operatively with clinical colleagues to ensure a cost effective and quality Breast Care Service.
* The post holder will organise own workload exercising a degree of personal and professional autonomy in the management of Breast Care patients.
* To liaise and contribute to multi-disciplinary discussions, to ensure the patient care pathway is coordinated and seamless.
* Establish and maintain effective communication with various individuals and groups on highly complex potentially stressful topics in a range of situations including supporting patients and their relatives/carers through the diagnosis, management and prognosis of breast cancer.
* To have an understanding of the Mental Capacity Act and Deprivation of Liberty and be able to apply to this in clinical practice.
* To have an understanding of safeguarding, being able to appropriately identify those patients at risk, managing referrals and assist with investigations in line with Trust Policy and Procedure.
* To provide support and care for the patient and his/her family respecting their need for privacy and dignity.
* Be professionally responsible for adherence to Trust policies and procedures.
* Produce written communications which are clear, fluent, concise and easily understood.
* Carry out nursing procedures and treatments in accordance with approved procedures.
* Ensure the delivery of evidence-based nursing care in accordance with NMC Code of Professional Conduct within the clinical care area as an autonomous practitioner.
* Apply current research findings to clinical care and actively disseminate these findings.
* Liaise closely local patient support groups and support the facilitation of breast cancer support groups.
* Attend and participate in service Breast Cancer meetings, conferences and professional forums locally and nationally to support best practice.
* To continue own professional development in line with NMC guidelines and NMC revalidation.
* To participate and support the process of Clinical Supervision within the Service.

Person Specification
Qualification and Professional Training
* NMC registration
* Degree level education
* Evidence of continual professional development
* Diploma Level qualification/training in speciality or equivalent
Experience and Knowledge
* Surgical experience which incorporates caring for patients who have undergone breast surgery or working within an oncology setting.
* Proven interest in working within breast care clinical setting or Oncology
* Post registration experience working as a band 5
* Evidence of multidisciplinary team working
* A knowledgeable clinician with the ability to apply evidence-based practice.
* Experience in teaching patients and staff
Skills and abilities
* Provide specialist advice to patients/ clients, carers in relation to condition
* IT and word processing skills
* Able to manage work autonomously
* Knowledge of professional and NHS issues, and policy relating to specialist area.
* Able to present information to professional groups
* Able to perform assessment, planning, implementation and evaluation of nursing care
* Able to participate/undertake clinical audit and research
* Skills and knowledge to undertake management of staff and patients within scope of practice or willingness to develop.
