Your role will be with adults over the age of 65 or of any age if a memory concern. It will involve assessment, care planning, implementation and evaluation of the care plan, in partnership with the client and family and carers. You will also be involved in assessment and management of ongoing risk. This role will also involve medication monitoring, liaising with psychiatrists, primary care and with other Social Care services, also referring onto other services as appropriate. You will be an active member of an MDT. You will be supervising students and support workers on occasion. The role will also include being part of the Place of Safety service for escort of detained patients to a psychiatric in-patient facility. You will be registered with the NMC and work within the code of conduct at all times. A driving licence is essential and a pool car may be available, however, applicants should have access to their own vehicle for work purposes, if required This is a permanent, part time post of 22.3 hours per week, to be worked in accordance with service requirements, plus participation in the on-call rota for the Place of Safety.
