The Community Psychiatric Nurse will be a member of the Older Adult Community Mental health team, working closely with the multi-disciplinary team. Responsibilities include maintaining clinical caseloads and conducting assessments of highly complex problems which require analysis, interpretation, and comparison of a range of solutions for service users experiencing severe and complex mental health difficulties, both organic and functional.
Main duties of the job
The Community Psychiatric Nurse will work within their code of practice to take responsibility for their allocated caseload to ensure assessments of health and social needs. This will include formulation, planning, and evaluation of care which includes regular reviews and updates of risk assessments documentation and care planning. They will be a care co-coordinator and advocate for the service user at times and promote independence through the recovery process.
The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.
