An exciting opportunity has arisen for a highly motivated individual to join our Specialist Nursing Team as a Parkinson's Nurse at Betsi Cadwaladr University Hospital Trust, working closely with Neurology, the care of the elderly team and the rehabilitation team.
Working closely with the team, the post holder will facilitate achievement of a quality patient experience, whilst acting within clearly defined policies/procedure guidelines, and codes of conduct.
This role will be based at Ysbyty Glan Clwyd. You will be provided with the support and equipment such as a mobile phone and laptop you will need to be able to fulfil this role.
The Parkinson's Nurse will offer support and advice to people with Parkinson's who are admitted to hospital, as well as offering assessments and reviews in a nurse led clinic in Ysbyty Glan Clwyd, with an emphasis on reducing admissions for people with Parkinson's.

Main duties of the job
The purpose of the Parkinson's disease Nurse Specialist (PDNS) is primarily to maintain and improve quality of life for people with Parkinson's disease (PwPD), provide support and education to PwPD and their families in any location, and support and provide a seamless service from diagnosis to palliative care.
The PDNS will act as an expert resource, providing advice, support and education for professional staff in the primary, secondary, voluntary and private care sectors. This will encompass the nursing care and management of people with Parkinson's disease (PD), working closely with patients, carers, consultants and general practitioners, therapists, social services and the voluntary sector advising on complex nursing care issues.
Highly specialist and comprehensive assessment of individual patient's needs will lead to the setting and implementing of programmes of care for patients, linking with primary care and hospital staff to ensure care is properly co-ordinated.
PDNS will promote and initiate research, evidence-based nursing practice and monitor the quality of the service being provided. Patient profiling will lead to an organised and identifiable caseload and ongoing PDNS self-resource evaluation will allow effective allocation of time.
New specialist support initiatives within the Hospital and Community will be developed and sustained by the PDNS.
