JOB PURPOSE:
Monday – Friday 0900-1700. Flexible. Full or Part time.
To plan, organize, and evaluate an innovative health visiting service, and where appropriate, delegate to other members of the health visiting team to ensure delivery of a high-quality service.
To provide a skilled, effective, and evidence-based health visiting service to clients in a variety of community settings.
To work in collaboration with other health care and service providers to ensure seamless delivery of services for clients and communities.
To collaborate with team colleagues in order to deliver the Quality Strategy objectives.
Deliver the healthy child programme and work in partnership with families to develop and agree tailored health plans as required.
Utilizing specialist skills and knowledge, advise and support individuals and families on evidence-based health care practices.
Understand the caseload profile including the health needs of the defined population and local communities, targeting outreach services to meet their needs.
Initiate referrals to multi-disciplinary/agency colleagues as required to ensure adequate care and support for families and children.
Assess, plan, implement, and evaluate individual/group programmes of care that meet the public health needs of the population.
To develop and delegate appropriately the implementation of regular health promotion initiatives on specific health issues and to specific groups and individuals.
CLCH NHS Trust is currently re-shaping the way Health Visitors work, looking at new and innovative practices to improve health across London. This includes agile working and provision of laptops to all staff.
To provide a skilled, effective, and evidence-based health visiting service to clients in a variety of community settings.
To apply specialist skills and knowledge in the delivery of the health visiting service.
To consult, advise, and educate individuals and families on good health care practices.
To develop health profiles of the defined population and local communities, identifying health needs of disadvantaged groups and targeting outreach services to meet their needs.
To identify, prioritize, and implement programmes of care that meets the public health needs of the population.
To provide parenting skills support and information both through coordinating/participating in parenting groups and one-to-one home visits to families, especially those children and families identified with complex health needs/vulnerabilities.
To provide health assessments, advice, and education to families and individuals of all ages in the population group, prioritizing services appropriate to local public health and population need.
To develop and implement regular health promotion initiatives on specific health issues.
