The Role
To provide pharmaceutical advice and lead and develop governance processes within the Hospital and Home service.
This exciting new role will contribute to the Hospital at Home Service across NHS Lanarkshire to ensure patients receive high-quality pharmaceutical care. The post holder will become an integrated member of the consultant-led multidisciplinary team. This post holder will be required to work across all 4 of the Hospital at Home bases at UHW, UHM, UHH and Clydesdale. There is a requirement for the post holder to be an established practising independent prescriber due to the complexities of medicines procurement, prescribing and administration within the service. The post holder will contribute to developing the clinical governance agenda and associated systems for medicine use within the service.

What we'll need you to bring
Masters of Pharmacy Degree
Registered member of General Pharmaceutical Council
Independent prescribing qualification or must work towards
Evidence of postgraduate training and professional development in clinical pharmacy practice e.g. Post Registration Foundation Framework/ 1st year clinical diploma
Relevant post registration experience in clinical pharmacy.
Ability to effectively monitor patients’ therapeutic regimens in order to ensure effectiveness and reduce toxicity
Ability to work effectively as part of a team, in a pharmacy and multi-disciplinary environment.
Commitment to revalidation as per directions of General Pharmaceutical Council
Good verbal and written communication skills
Self-motivation
Participation in weekends and public holiday service
Participation in pharmacy emergency duty service
Willingness to contribute to the general working of the pharmacy department.

Please note this is a fixed term post for 9 months. Where a post only has temporary funding and an existing member of NHS Lanarkshire staff wishes to apply, this will be treated as a secondment. The employee MUST already have written agreement from their line manager to be released on a secondment before applying for the post.
Location and Working Pattern
This role will be based in University Hospital Monklands
The working pattern for this role Mon-Fri; 9:00-5:00.
