About the role

Our Hospital Social Work Team provides timely and robust Social Care Act assessments for individuals following hospital discharge who may have social care needs. We work with adults aged 18 and over, supporting those with complex needs—whether they are new to the Local Authority or require continued care. This includes community care packages, placements, step-down beds, and Best Interest decisions for those at risk.
Our team operates Monday to Friday, 9:00 AM – 5:00 PM from the Civic Centre in Uxbridge, with out-of-hours support provided by the Emergency Duty Team.
About You
We are looking for an experienced and knowledgeable Senior Social Worker who is motivated to drive positive change for residents and their families. You will be a role model for best practices, supporting and mentoring others while managing a varied caseload independently.
To succeed in this role, you must have:
* A Social Work qualification and registration with Social Work England.
* At least 12 months of post-qualification experience, including completion of the ASYE, where applicable.
* Strong knowledge and experience working in Hospital Social Work.
* A proactive, team-oriented approach, with the ability to work independently and manage competing priorities.
