The Role

We currently have positions in our Neighbourhood Teams. It’s an exciting time to come and work for us as you will be joining the team at a time of transformation and great investment into services for adults and social work practice.
About You
You will take the lead on supporting adults to live well at home. You will be part of a friendly and supportive team and your work will include managing a caseload, Care Act Assessments, Support Plans, Mental Capacity Assessments and Best Interest Decisions and signposting to internal and external services where necessary to enhance wellbeing and independence.
You will have excellent communication skills and have the ability to build effective professional relationships with other disciplines and with other local health and social care services to enable a coordinated response to individuals and families.
