Based at Tilehurst Clinic, Reading, and working 30 hours flexibly over the week, this is an exciting permanent Dietetic Assistant Practitioner (DAP) post. The role will combine working with patients on Home Enteral Nutrition tube feeding (HEN) in their own homes and nursing homes across Reading & West Berkshire andprovidingnutrition support advice to inpatients on our Community Inpatient units.
The role suits someone with experience and/or knowledge of clinical nutrition as well as a desire to improve the nutrition of the local population and support vulnerable patient groups.Experience in the NHS or ahealth / socialcare setting is an advantage.
The role involves working alongside registered dietitians toprovidetailored nutrition support advice to patients, visiting patients in their own homes or nursing homes to review andmonitortreatment plans, ordering equipment, day-to-day administration support and problem-solving to improvedepartmentprocesses.
To support registered dietitians with dietetic management of delegated Home Enteral Nutrition (HEN) patients in their own homes and nursing homes, and nutrition support (NS) patients in community inpatient wards.
• To manage a defined caseload of HEN and NS patients under the guidance of a registered Dietitian.
• To implement and review dietetic treatment plans for HEN and NS patients under the care of the dietetic service.
• To assess, plan, implement and evaluate treatment plans for delegated patients.
• To review patients on delegated HEN and NS caseload: Use judgment to review aims,modifyand adapt treatment plans
• To collect and assess audit data to helpmonitorusage of equipment and implement changes to practice whereappropriate.
