This Complex Discharge Facilitator role will be based in the Integrated Discharge Hub and you will be required to support the ward based Patient Journey Facilitators to facilitate the SAFER Red2Green approach and also to assist the IDT Hub with the daily discharges.
SAFER Red2Green aims to value patient time and improve flow by reducing delays in a patient’s hospital journey.
We are therefore seeking a keen individual with a ‘can-do’ attitude who is able to work alongside the integrated health and social teams and the Patient Journey Facilitators to progress the patient’s journey.
The successful candidate will need to be proactive and personable and able to work under pressure to meet tight deadlines. Ideally, you should have an understanding of SAFER Red2Green approach and the different pathways to ensure the patients’ journey is progressed in a timely manner.
Inform ward based Patient Journey Facilitators and Ward Co-ordinators of updates and relevant information to ensure patients’ are discharged in a safe and timely manner.
Daily update of database with regards to the failed discharges across both sites.
Actively seeking to ensure that all patients are managed through their pathway. Where this is not likely to be met; to proactively take steps to ensure that patients investigation and treatment schedules are brought back within target.
To understand the pathway of each individual patient on the Complex Worklist; monitoring their progress and ensuring timely intervention is achieved.
Attend daily Integrated Discharge Team (IDT) team meetings, and identify the patients who require the support of social services and or CCG to facilitate discharge.
Ensure the accuracy of the information entered on the Computerised SEMA system and that PSAG Boards are updated followed with plans and actions.
The Complex Discharge Facilitator role is vital in supporting the wards to ensure patients are discharged in a safe and timely manner.
The Complex Discharge Facilitator sits within the Capacity Centre, along with the Clinical Site Managers and provide support to the wards in respect to the patients’ journey and their discharge process.
This is a very busy and proactive role, where no two days are the same. The role is very diverse and they work closely with a range of multi-disciplinary teams to ensure that patient pathway and planned discharge runs smoothly and effective.
