Band 7 Community Matron Homeless pathway - 37.5 hours per week
North East London Foundation Trust has an exciting opportunity for a Community Matron Homeless pathway in Waltham Forest, ICT Central, Integrated Community Team.

Main duties of the job
The aims and intended outcomes of the service are based on the commitments in the ‘Healthcare and people who are homeless Commissioning Guidance for London’, which was produced by Healthy London Partnership (HLP) revised April 2019.
The service provider will carry out a triage and a comprehensive physical health check for all residents that consent to this assessment. Based on the outcome of this health check, a plan will be put in place to address any unmet needs or signpost the resident accordingly. All residents found not to be registered with a GP will be supported in registering with one and accessing other health services or social care/voluntary sector support.
The post holder will be a Registered Nurse, with at least 2 years community experience of managing a complex caseload of patients. You will be responsible for a caseload of patients with complex health and social care needs. You will work autonomously to case manage these clients and use advanced nursing skills, such as assessment and prescribing to manage their conditions in the community.
They will work as part of the integrated care team, liaising with GPs, other professionals and statutory and voluntary agencies to deliver individualised care to patients.

Starting with NELFT
NELFT places a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment.
High Cost Area Supplement - Outer London
This post also attracts payment for High Cost Area Supplement– 15% of basic salary, with a minimum of £4,551 and maximum of £5,735, pro rata for part-time staff.

You will manage a caseload to maximize efficacy and effectiveness in your role in order to provide the best service to the patient.
You will utilize a range of quality improvement tools to review professional practice and initiate and evaluate changes to practice when required.
You will access Clinical Supervision and professional support from sources agreed with the line manager to support critical reflection and safe practice.
You will support and supervise colleagues and provide targeted education, training and development as required.
You will engage positively in performance management and participate in the review and development of your Personal Development Plan as required.
You will demonstrate effective clinical and professional leadership in regards to developing the strategy for the development of services surrounding Long Term Conditions.
