Job summary The Community Mental Health Framework describes how the Long Term Plan's vision for a place-based community mental health model can be realised, and how community services should modernise to offer whole-person, whole-population health approaches, aligned with the new Primary Care Networks. NHS England has set out a plan of investment to deliver the CMHF in Derbyshire. Derbyshire has been allocated £15m investment which has been split between DHCFT, Local Authority and Third Sector. Main duties of the job Provide strategic oversight of Living Well and the Additional Roles Reimbursement Scheme (ARRS), take a lead role in the transformation Programme Team to provide oversight and reporting of the ongoing development of the programme along with embedding the work undertaken to date. Supports key partners in Derbyshire to drive transformation, support and lead in the development of pathways of care across the Trust and Primary care, and implement and embed new ways of working Work across the interface between Primary Care and Secondary Mental Health Services to deliver an integrated model of community mental health care. Building strong relationships with the PCN's, Living Well Teams, and wider stakeholders, to support the successful integration of ARRS mental health practitioners, along with developing and maintaining strong relationships across the ICS to support the phased mobilisation of the Living Well Teams. Demonstrate & nurture a culture change within services as outlined in the Community Mental Health Framework, to ensure services are person centred, needs led, co-produced, community inclusive and place-based.
