As a pharmacist, you will play a crucial role in supporting our clinical team by ensuring the safe, effective, and efficient use of medicines. You will work closely with GPs, Clinical Pharmacist, Nurses, and other healthcare professionals to provide comprehensive pharmaceutical care to our patients.
Main duties of the job
The post-holder will look to improve patients' health outcomes and the efficiency of The Lennard Surgery by providing direct, accessible, and timely medicines expertise. Pro-actively transferring workload relating to medicines optimisation issues from other clinical staff to improve patient care and safety, you will be freeing up other clinical staff time to enable them to meet the needs of more patients with clinically complex needs.
Job Responsibilities
Primary responsibilities include:
1. Work as part of a multi-disciplinary team in a patient-facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.
2. Be a prescriber, or completing training to become a prescriber, and work with and alongside the general practice team.
3. Be responsible for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, those with multiple co-morbidities.
4. Provide specialist expertise in the use of medicines whilst helping to address both the public health and social care needs of patients at the organisation and to help in tackling inequalities.
5. Provide leadership on person-centred medicines optimisation and quality improvement, whilst contributing to the Quality and Outcomes Framework and enhanced services.
6. Through structured medication reviews, support patients to take their medications to get the best from them, reduce waste and promote self-care.
7. Develop relationships and work closely with other pharmacy professionals across the wider health and social care system.
8. Act as the point of contact for all medicine-related matters, establishing positive working relationships.
9. Consult patients within defined levels of competence and independently prescribe acute and repeat medication.
10. Manage a caseload of complex patients and potential care institutions and provide advice for the GP management of more complex patients.
Person Specification
Experience
* Ability to communicate complex and sensitive information effectively with people at all levels by telephone, email, and face to face.
* Excellent interpersonal, influencing, negotiation, and organisational skills.
* Knowledge of IT systems, including ability to use word processing skills, emails, and the internet.
* Effective time management (planning and organising).
* Demonstrate personal accountability and emotional resilience, and work well under pressure.
* Good clinical system IT knowledge of EMIS.
Qualifications
* GPhC registered pharmacist.
* Ability to follow legal, ethical, professional and organisational policies/procedures and codes of conduct.
* Hold or be working towards a GPhC independent prescribing qualification.
