You will have excellent communication and interpersonal skills with a passion for delivering high quality, evidence-based care to our patients.
Experience with chronic disease management, women's health, child and adult immunisation and health promotion is desirable.
You will be responsible for the delivery of practice nursing services, working as part of the practice's multidisciplinary team who provide care within their scope of practice to the entitled patient population. The practice nurse will be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention, well woman and well man clinics, as well as supporting the management team in the reviewing of clinical policy and procedure.

Main duties of the job
Alongside the GPs, the Nursing Team deliver clinical evidence-based practice for our local population, meeting the needs of the patients with particular attention to patients with long term conditions, and the provision of preventative healthcare. The ideal candidate should have experience of working in areas of Chronic Disease Management, Cervical Cytology and vaccinations/immunisations. In addition to this, the candidate should have experience of providing routine healthcare assistance, wound care, implementing well woman and well man clinics, assisting GPs with minor surgery, supporting the team with clinical emergencies, and recognising, assessing and referring patients presenting with mental health needs.

We are a well-respected and long established semi-rural practice in the historic town of Arundel, lying at the foot of The South Downs National Park. Arundel Surgery is a well-organised, supportive close-knit team consisting of three GP Partners, one GP retainer, and an experienced practitioner team comprising an ANP and two Paramedic Practitioners. We have a highly skilled Practice Nursing Team and efficient Patient Care Navigators and Administrative Team who run a well-organised workflow system. We are supported by a First Contact MSK Practitioner, Clinical Pharmacist, Pharmacy Technician, Mental Health Practitioner, and Care Coordinator.

Job responsibilities
The following are the core responsibilities of the practice nurse:
1. Develop, implement and embed health promotion and well-being programmes.
2. Implement and evaluate individual treatment plans for chronic disease patients.
3. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects on patients' health.
4. Provide routine nursing care to patients as required in accordance with clinical-based evidence, NICE and the NSF.
5. Provide wound care (ulcer/Doppler etc.) to patients.
6. Request pathology services as necessary.
7. Process pathology results as required.
8. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required.
9. Maintain accurate clinical records in conjunction with extant legislation.
10. Ensure read codes are used effectively.
11. Maintain chronic disease registers.
12. Develop, implement and embed well woman clinics.
13. Develop, implement and embed well man clinics.
14. Chaperone patients where necessary.
15. Assist GPs with minor surgery when required.
16. Prioritise health issues and intervene appropriately.
17. Support the team in dealing with clinical emergencies.
18. Recognise, assess and refer patients presenting with mental health needs.
19. Implement vaccination programmes for adults and children.
20. Support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice).
21. Liase with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.).
22. Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual).
23. Support the clinical team with all safeguarding matters, in accordance with local and national policies.
24. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately.
25. Deliver opportunistic health promotion where appropriate.
