We are seeking a highly motivated professional who will provide care delivered autonomously within their clinical competencies but from within a supportive multi-disciplinary team. They will provide expertise in clinical medicines optimisation across the Primary Care Network. Main duties of the job Provide professional input to PCN Practice teams & network multidisciplinary teams working to improve patient outcomes and reduce avoidable admission to hospital. Ensure optimum benefit for prescribed medication through high levels of compliance & concordance achieved by support for the patient and carers. Reconcile medicines following hospital discharge, amend medication lists & contact patients/ carers regarding such changes, as appropriate, to ensure patients receive the medicines they need post-discharge. Provide direct care to patients with long term conditions, minor ailments & other clinical areas within the scope of practice. Review MHRA alerts, organise appropriate searches/audits, implement drug withdrawals, etc; improving medicines safety. Support the care of complex patients in their own homes or in nursing or residential accommodation as part of a multidisciplinary team, with an option to undertake care home optimisation work. Provide professional leadership on matters relating to medicines optimisation and quality improvement. Lead on the prescribing domains of the Quality & Outcome and Consolidated Funding Frameworks.
