An excellent opportunity has arisen for a motivated and enthusiastic Discharge Facilitator to support the Thoracic Surgery Day 1 discharge programme at South Tees Foundation Trust.
The successful applicant will ideally have previous experience working in a ward setting, specifically supporting AHP teams and is expected to have previous experience of supporting discharge planning.
The Physiotherapy Service is a well-established large, progressive and dynamic service, which continues to develop within the Trust. We have an opportunity to explore a comprehensive Day 1 Discharge service where exceptional planning, communication and commitment will be key to its success.
Main duties of the job
The post holder will be recruited on a 12 month fixed term contract and will be employed as a Day 1 Discharge Facilitator, supporting the Day 1 Discharge Lead for patients who have undergone thoracic surgery.
The post holder will provide clinical assessment and support with the rehabilitation and physiotherapy management of the thoracic surgical patients. Alongside the existing thoracic team, they will contribute to clinical developments and research opportunities to proactively move the thoracic physiotherapy service forward to the benefit of patients and staff within the Trust and at a regional network level.

Qualification and training
* Have or working towards a relevant Foundation Degree (if available), relevant other first degree but without professional qualification, or NVQ4, NVQ3 WITH additional accredited training in the specialist field or equivalent evidence of underpinning knowledge
Knowledge and skills
* Can demonstrate understanding and awareness of the Discharge facilitator role
* Practical and theoretical knowledge of patient conditions and treatment within relevant specialty
* Substantial previous experience as an AHP Assistant
* Knowledge and working experience in A&E and FOH wards
* Experience at educating and training assistants and qualified therapists within clinical area of expertise
