Job summary A fantastic opportunity has arisen for a highly specialist B7 nurse to join the existing chronic kidney failure team in Ysbyty Gwynedd. The successful post holder will be required to manage complex renal failure Patients living with kidney disease.The postholder will be required to fully integrate with our other renal services to ensure care is provided at the point of patient need. Our key aim is to keep patients well at home to assist in avoiding hospital admissions where possible. Facilitating difficult conversations when the need arises to discuss patient modalities of care.You will be required to have advanced skills such as clinical history taking, non-medical prescribing, clinical management of these patients will be delivered to a high standard.You will also support the renal team in delivering training and education in all care settings. Excellent clinical and communication skills are essential requirements for this post.To assist in sustaining and development of a service to improve the health and wellbeing of individuals living with chronic kidney disease. Main duties of the job To be able to act autonomously in the assessment and management of kidney failure patients using protocols, policies, clinical supervision and clinical judgement to guide practice. While working collaboratively alongside the wider multidisciplinary renal team.The ability to develop and implement individual care plans ensuring patients receive appropriate treatment and support.Encompassing education and symptom management assisting the patients and their family's to live well with kidney disease.Monitoring of the patients under the care of the renal team, escalating care needs when necessary.Facilitating nurse led clinics, care coordination alongside the already established chronic kidney care team.
