Team Manager – Specialised Disability Services

We are currently looking for a Team Manager to lead the development of a new service for Ceredigion that will focus on providing specialised Disability Services. The service will support individuals with care and support assessments and complex disabilities, including Learning Disabilities.
The service is part of the Ceredigion Through Age Model and will work with all ages, particularly focusing on the transition from childhood to adulthood and beyond.
The position will play a fundamental role in supporting a change management process, moving from traditional services to the new model through a collaborative approach with colleagues from across the authority and partner agencies, e.g., Health.
