The post holder is responsible for the delivery of general practice nursing to the whole practice population. Supported by the GPs within the practice, they will recognise and work within their own competence and professional code of conduct as regulated by the NMC. The post holder will make an assessment of the patients needs and act upon their assessment. Produce accurate, contemporaneous and complete records of patient consultations, consistent with legislation, policies and procedures. They will act collaboratively with the general practice team to meet the needs of patients by providing quality evidence based care. They will support the delivery of policy and procedures and will be responsible for maintaining a personal development plan. The post holder willhave a responsibility to support the practice in achieving its targets for, chronic disease management, public health initiatives, LES, DES and QOF. It is essential that you ensure patients health checks and records are up to date at all times. Where patients are requiring periodic checks for chronic disease you are required to carry out those checks and to ensure that any other checks which are due within a short timescale are also carried out so as to improve lean working within practice and improve patient experience, by reducing number of times they have to come to the surgery
