Social Care Worker - Rehabilitation Support

What we do:
The Social Care Worker – Rehabilitation Support is a key part of the Intermediate Care Team, delivered jointly by NHS Shetland and Shetland Islands Council. The team includes a Nurse Team Manager, Occupational Therapists, a Social Care Worker Practitioner – Rehabilitation Support, and other Social Care Workers. Referrals are made by professionals involved in the individual's care, such as social workers, GPs, community nurses, or physiotherapists.
The team provides short-term support and reablement services to increase independence, reduce hospital or care home stays, and prevent unnecessary hospital admissions. They assist service users in setting achievable SMART goals and support them in working towards these goals. The team also offers a Community Falls Risk Screening service.
What you'll do:
As a Social Care Worker – Rehabilitation Support, you will work with the multidisciplinary team, service users, and their families or carers to plan, develop, implement, and evaluate rehabilitation services in various settings, including the service users' homes. * The pay rate includes a Distant Islands Allowance of £1.4808 per hour
* Permanent part-time role, 30 hours/week, 52.18 weeks/year
* Candidates must join or update the PVG Scheme before employment
* Registration with the Scottish Social Services Council (SSSC) is required upon starting
* This role does not qualify for UK visa sponsorship
* No relocation package available
* Interview expenses will be paid within Shetland
* Must be able to travel across Shetland at short notice, with limited public transport
