To provide assessment, planning, implementation and evaluation of interventions for people presenting with a severe and enduring mental health problem alongside Dual Diagnosis within a designated geographical area or as required by the ServiceTo work within the principles of the Trusts Dual Diagnosis Policy and to help with early identification of people who might benefit from the service and or who might be diagnosable with coexisting disordersTo act as a key worker for a caseload of, assessing their needs, and developing their recovery care plan, Dialog plus, coordinating and reviewing their care and treatment outlined in the Dialog plusTo coordinate working practices amongst other relevant professionals in the designated locality, including psychologists and psychological therapistsTo develop and maintain ones own clinical skills and expertise in the area of approaches to, and treatment of people with a mental health difficulty, Dual Diagnosis and, to be reflective and responsive to meaningful service user and carer involvement.To be part of the teams duty rota
