Engagement Lead and Health Check Trainer
You will work flexibly across the borough of Dudley, engaging within a range of communities and partners. Delivering health checks to the general public between the ages of 40 and 74, particularly those from the most marginalized groups, within community and clinical settings.
You will lead on training and competency checks for existing staff and partner agencies to deliver health checks, which may include working with partners outside of the Dudley borough.

Duties and Responsibilities
Key responsibilities will include:
1. Ensure the NHS Health Checks service is of high profile and there is strong awareness in relation to the criteria of entry into the service.
2. Assist the Head of Service to develop the integrated health offer within Dudley and raise the profile of ABL Health.
3. Identify and engage with individuals from target groups and communities to facilitate communication regarding promoting their health and wellbeing, and the Lifestyle service.
4. Attend health events across the borough, undertake health checks, and promote the wider Dudley service.
5. Develop and maintain relationships with individuals experiencing the greatest inequalities in health and provide information about the relationship between behaviors and health.
6. Make contacts and develop professional links within communities, promoting the Health Checks Service and your role to contact groups where there may be barriers to understanding.
7. Liaise with key partner organizations such as workplaces, community organizations, community centers, and faith groups to increase referrals into Your Health Dudley services.
8. Support all Your Health Dudley services to ensure quality and governance are robust.
9. Promote the integrated Lifestyle Service and all components within the local community.
10. Champion and adhere to processes and procedures to identify and strive for continuing improved quality throughout the patients' pathway, together with robust data management.
11. Deliver against an agreed set of KPIs.
12. Meet internal and external performance management requirements.
13. Ensure rigorous and regular data inputting to maintain accurate records.
14. Work within the requirements of the 6 Cs: Care, Compassion, Competence, Communication, Courage, and Commitment.
15. Explore partnership opportunities and co-production activities.
16. Train existing and new staff to deliver Health Checks.
17. Conduct annual competency and observation checks for all staff delivering Health Checks.
18. Attend local health forums/groups/meetings to represent ABL, promote the service, and maintain local partnerships.
19. Support the Head of Service to develop robust SOPs.
20. Support the Head of Service to complete 10% audits of all work carried out.
21. Engage with GP surgeries and Primary Care Networks to develop case finding and population health management projects to drive referrals into all elements of the service.
22. Develop partnerships with secondary care and inpatient services to increase smoking cessation referrals.
Skills and Competencies Required
Embed referral pathways to ensure efficiency and quality referrals into the service.
