To work in partnership with parents/guardians and gain informed consent. To create individual care plans for specific children, negotiating, agreeing, implementing and evaluating with parents and other professionals. To be responsible for assessing, delivering, developing and maintaining high standards in the quality of care in the home and clinic setting. Delivering face to face support and/or telephone advice. To work closely with the GPs and respiratory consultants to develop effective pathways of care and clinical management plans for patients. To maintain skills and training for procedures. Maintain an update log of all training undertaken. Identify and respond to issues of clinical risk. Provide information and training to parents based on assessment of need. Participate and support regular educational audit demonstrating example on an annual basis. To assist in the education and practical development training of all learners, utilising every opportunity for clinical teaching. To develop protocols and guidelines to ensure patient centred, age appropriate safe management, for children and young people with asthma. This will include the transition to adult services. To recognise own professional limitations Demonstrates the ability to manage time and resources effectively. Demonstrate an ability to prioritise workload and is able to show flexibility in response to urgent and/or changing needs. Through reflection and clinical support, develop strategies to improve, and where necessary change practice. Identify issues relevant to practice governance and participate in sharing knowledge in this forum. Identify areas of practice development and demonstrate the implementation of one practice development initiative per year. In addition to the basic salary, this post attracts a High Cost Area Supplement of 5% of basic salary subject to a minimum payment of £1,066 and a maximum payment of £1,845 per annum (pro rata where applicable).
