Advanced Nurse Practitioner - Moorlands Rural PCN
To work as an autonomous practitioner, as part of the practice multidisciplinary team, delivering care within their scope of practice to the patient population. Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice to patients.
The advanced nurse practitioner will be responsible for a number of clinical areas such as health promotion, minor illness, as well as supporting the management team in reviewing clinical policy and procedure.

Although you will be part of the Moorlands Rural PCN team, you will mainly be based at Werrington Village Surgery. We are a caring, friendly, and supportive practice looking to recruit a dedicated team member to help maintain our high standard of patient care. We are a forward-thinking training practice caring for 8,500 patients. The practice has a village feel and is supported by an active Patient Group.
Job Responsibilities
Primary Responsibilities
1. Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice to patients.
2. Develop, implement, and embed health promotion and well-being programmes.
3. Implement and evaluate individual specialised treatment plans for chronic disease patients.
4. Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on their health.
5. Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE, and the NSF.
6. Request pathology services as necessary.
7. Process and interpret pathology and other test results as required.
8. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required.
9. Maintain accurate clinical records in conjunction with extant legislation.
10. Ensure SnoMed codes are used effectively.
11. Prioritise health issues and intervene appropriately.
12. Support the team in dealing with clinical emergencies.
13. Recognise, assess, and refer patients presenting with mental health needs.
14. Extended and supplementary prescriber, adhering to extant guidance.
15. Support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice), reviewing annually as required.
16. Contribute to practice targets (QOF etc.), complying with local and regional guidance.
17. Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual).
18. Support the clinical team with all safeguarding matters, in accordance with local and national policies.
19. Deliver opportunistic health promotion where appropriate.

* Registered Nurse
* Post graduate diploma or degree (Advanced Practice Qualification)

* Experience of working in a primary care environment
* Experience of prescribing and undertaking medication reviews
* Experience of working as a practice nurse or community nurse
