Dashwood pharmacy team is growing and we have an exciting opportunity for another Clinical Pharmacist to join us.
Main duties of the job
We are looking for someone enthusiastic about providing high quality care to our patients and becoming an integral part of our growing clinical pharmacy team.
The Clinical Pharmacist will be a patient facing role with plenty of opportunity to utilise your clinical skills to improve the health of the local community; this will include managing long-term conditions, supporting hospital discharge prescribing arrangements, providing specific advice for those on multiple medications, clinical audits to improve patient care, and supporting the GP practices involved to develop the clinical pharmacist role.

KEY RESPONSIBILITIES
6. Work as part of a multidisciplinary team in a patient-facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.
7. Take responsibility for areas of chronic disease management and undertake structured medication reviews to proactively manage patients with complex poly-pharmacy.
8. Develop and manage a medicines management plan and deliver patient services as determined by Network policy and local and national guidance.
9. Maximize cost-effective prescribing and improve the quality of patient
10. Provide specialist expertise in the use of medicines whilst helping to address both the public health and social care needs of patients in the PCNs practices and to help tackling inequalities.
11. Develop relationships and work closely with other pharmacy professionals across Primary Care Networks and the wider health and social care system.
12. Take a central role in the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists (including mental health and reduction of inappropriate antipsychotic use in people with learning difficulties) liaison with community pharmacists and anticoagulation.

20. Ability to define, collate, analyse and interpret data

23. Masters Degree in Pharmacy MPharm or equivalent
24. Registered with GPhC
25. Member of RPS
Experience

26. Specialist knowledge through a Postgraduate Diploma and qualifications (eg clinical, community, therapeutics) or equivalent
27. Evidence of recent and relevant Continuing Professional Development
28. At least 2 years post registration experience in a hospital, community or general practice setting
29. Experience of undertaking medication or medicine use reviews, and patient counselling
30. Experience of working with clinicians and multidisciplinary/multiagency work
31. Experience of facilitating change to improve clinical practice
32. Experience of delivering training sessions to varied groups
33. Experience in undertaking clinical audit

34. Independent prescriber status or a commitment to undertake the course
