The shift pattern for this position is Monday - Friday.
We are looking for a compassionate and supportive Occupational Therapist (Band 6) to join our dedicated team in the Community Hospital Discharge Team. Our multidisciplinary team includes Physiotherapists, Occupational Therapists, Nurses, OT Assistants and Rehabilitation Support Workers, all working together to provide therapy assessment and intervention for adults who have recently been discharged from hospital.
About the Role:
As a Band 6 Occupational Therapist within CHDT, you will be clinically responsible for a caseload of patients, working closely with the team to ensure individuals can safely remain at home, avoiding unnecessary hospital re-admissions. You will play a key role in delivering high-quality care, supporting the patient's recovery and independence in their own home environment.
Key Responsibilities:
1. Manage a caseload of patients/clients, providing care and interventions to enable them to stay at home, considering people in the context of their physical and social environments and identifying solutions that reduce or remove barriers to participation in their home and communities.
2. Contribute to multidisciplinary team working, ensuring integrated care for patients.
3. Provide supervision and mentoring to OT Assistant and Rehab Support Workers.
Essential Skills and Qualifications:
4. Registered Occupational Therapist with HCPC
5. Experience in a community setting or similar role is desirable.
6. Strong clinical decision-making and assessment skills.
7. Ability to work independently and as part of a multidisciplinary team.
