An exciting opportunity has arisen for an enthusiastic and experienced Nurse / AHP to join the Kerrier Homefirst team, based in Camborne Health Office.
This position is open to Nurses, Occupational Therapists, and Physiotherapists who have experience of working in the community and wish to develop their assessment, care planning, and reablement skills whilst being part of a dynamic, progressive, integrated multiagency, multidisciplinary team.
The Homefirst team is part of the integrated community team in Kerrier comprising of registered and non-registered nursing, therapy, and administrative staff working collaboratively with partner organisations to support the intermediate care needs of our local population.
You will be responsible for assessment, planning, and implementation of care and assist in the interpretation of information relevant to the patient's condition/progress, identify problems and needs of the patient, and set realistic goals. You will need to have excellent organisational skills combined with a wide knowledge base and experience of working in older person's services.
This vacancy provides an opportunity to develop and widen your skill set to include a holistic and clinical approach to patient care within the community. This post does involve regular weekend working and the successful candidate must be flexible with their working days.
Main duties of the job
* To provide high standard Occupational, Physiotherapy, and Nursing services, performing advanced assessments of patients including those with highly complex presentations, and from this, formulating a diagnosis and treatment plan.
* To operate as an advanced practitioner being the first contact for urgent requests to help both prevent hospital admission and facilitate early discharge.
* To utilise advanced skills in community assessment and management of the acutely sick patient.
* To provide leadership for support staff and junior staff through supervision, training, and appraisal.
* To participate in the planning and development of occupational services within the designated team.
* Model and deliver excellent evidence-based clinical care, working as an autonomous Practitioner.

Person Specification
Education/Qualifications
* Degree or diploma in Occupational Therapy or Physiotherapy
* HCPC registered, NMC Registered
* Evidence of appropriate postgraduate courses and CPD to support practice, to degree equivalent level
Knowledge and Abilities
* Confident and competent leadership with the ability to work in diverse and complex situations
* Proven leadership qualities, including the ability to motivate a team and negotiation skills
* Ability to make clinical risk-management decisions in lone-working situations
* Awareness of and ability to respond to the pressure of political change in health and social care systems
Skills and Aptitude
* Clear understanding of clinical effectiveness and the need to provide evidence-based care
* Highly developed verbal and written communications skills that will enable the post holder to work effectively across boundaries and in a variety of settings
* Imparting bad news to patients and their families/carers; demonstrating the ability to communicate in a form and manner that is consistent with the level of understanding, culture, background, and preferred communications method of each individual
* IT Literate
* Teaching, Assessing, coaching, and mentoring skills to facilitate an effective learning environment for staff and students
Experience
* Substantial post-registration clinical experience in a similar role
* A broad range of clinical experience in a variety of community and inpatient settings
* Extensive experience of working within a multidisciplinary team
* Experience of managing the risks associated with working in a lone working environment
