Job summary Community Unplanned Care Service Lead Hours: 37.5 per week (part time considered) Location: Peace Hospice, Watford Salary: Rennie Grove Peace Band 7 - £46,148 to £52809 per annum (pro rata) Closing date: 2 May 2025 at 12 noon Interview date: 16 May 2025 The purpose of the role is to provide leadership and high quality specialist palliative practitioner care, skills, advice and support to patients, carers and those important to the patient. This role will directly manage a team of band 6 nurses, paramedics and an HCA. The role supports the delivery of planned care within a virtual ward model and responsive visits to support patient care across Rennie Grove Peace Hospice (RGP). The virtual ward is based at Peace Building in Watford with a plan to further expand the virtual ward across the RGP NW Herts community locality. Main duties of the job This means you will be responsible to: Actively participate in the development of the organisations planned and unplanned community model of care. Offer advice, support and education to the primary health care team as part of an interdisciplinary team. The management of a specialist palliative virtual ward caseload across the RGP Watford, 3 Rivers and Hertsmere locality and support other RGP virtual wards within Hertfordshire as required to maintain our service. Demonstrate specialist skills and expertise in palliative care, the post holder will deliver high standards of personalised care.
