About the role
We are seeking a values-driven and proactive Case Manager to assist referred clients to address issues exacerbating their risk of homelessness. The overall outcomes are to reduce ongoing homelessness and re-offending among court support participants.
Reporting to the Team Leader, this is a permanent, full-time position based in Coburg North, Victoria. Salary and conditions are in accordance with the SCHADS Award Level 5.
You will successfully
1. Ensure that delivery of services and support is aligned with the National Homelessness Stream Model of Care.
2. Assess the immediate and long-term housing needs of service users who are at risk of homelessness upon release on bail.
3. Explore and connect service users to appropriate housing options.
4. Support clients to develop an individual case plan and monitor their journey towards goals.
5. Identify new networking opportunities, and build and maintain relationships with services and agencies.
6. Using a trauma-informed model of care, provide outreach and case management to clients referred from Court Services Victoria.
7. Ensure safety and complete risk assessments routinely.
You will have
1. Tertiary qualification in Social Work or related community service field.
2. A current Victorian Employee Working with Children Check.
3. Current Victorian Driver's Licence.
4. The ability to understand and respond to the needs of individuals experiencing homelessness.
5. Knowledge of the homelessness sector, including principles, policies, and legislation.
6. An understanding of homelessness models and approaches.
7. Knowledge of the Justice system and experience within correctional settings is desirable.
8. Experience in working with a range of stakeholders and networks.
