Tezlom are recruiting a Floating/Outreach Support Worker to provide people with flexible packages of support.
The service can provide a variety of supported living arrangements, including: individual homes, shared supported houses, extra care services, and support within the community.
Flexibility and choice
1. Developing domestic and life skills (housekeeping, shopping, cooking, cleaning, personal care, etc.);
2. Taking medication;
3. Managing a caseload, coordinating, and delivering assessment and evidence-based interventions (brief and structured), recovery planning, and aftercare planning;
4. Managing your money, including paying bills;
5. Getting out and about in your community;
6. Support to attend college, work placements, and other activities;
7. Accompaniment to appointments;
8. Socializing and meeting new people;
9. Help accessing other services;
10. Travel training;
11. Keeping safe and healthy;
12. Filling out forms; and much more.
However, it is important to remember that this is not a complete list, and there will be people needing support who do not fit neatly into these groups.
You will need to focus on getting to know people - how they want to be supported, what they want to achieve in life, and what is important to them - to offer support that is as individual as they are.
