Job Summary
Single Point of Access is looking for an experienced nurse who is highly motivated and has excellent communication skills, to join our team based in Ellesmere Port.
Working as a Nurse Coordinator, the post holder will effectively manage referrals from a variety of care settings and utilise various care pathways, ensuring that patients are referred to the most appropriate service for treatment.
This is a non-patient facing role working within a call centre setting.
This vacancy is Fixed term: 12 months, Part time - 28 hours per week.
* We offer a relocation package of up to £8,000. The relocation package is for anyone who would need to relocate in order to take up employment with Cheshire and Wirral Partnership.
Main duties of the job:
The post holder will be responsible for facilitating the management of patients with unplanned care needs by ensuring that patients are referred to the most appropriate service which meets their individual health needs, adopting a patient centred approach. This includes admission to hospital where appropriate, preventing unnecessary hospital admissions by utilising community services and facilitating a smooth transition of care between primary care, secondary care and community care settings.

Qualifications
Essential:
* Registered General Nurse (RGN)
Desirable:
* Post-registration qualification in relevant subject
* Previous experience of facilitating clinical supervision
Knowledge and Expertise
Essential:
* Extensive current clinical knowledge
* Evidence of ongoing professional development
* Excellent communication skills, verbal and written
Experience
Essential:
* A Registered Nurse Adult, currently practising with post registration experience within a community/primary care setting
* Leadership / management skills
* IT literate with good keyboard skills
Desirable:
* Experience of triage / multidisciplinary / interagency working
