The role will primarily focus on: Conducting home visits for frail, elderly, and housebound patients, ensuring timely assessments and treatment. Providing urgent and routine care to patients in their homes, including chronic disease management, end-of-life care, and post-hospital discharge reviews. Supporting the wider practice team with urgent care, minor illness management, and long-term condition reviews within the practice. Working closely with GPs, community teams, and social care services to ensure seamless patient care. About the Candidate If you are an experienced emergency care practitioner (ECP)/paramedic wanting a new challenge, we would love to hear from you. You will have the following skills and experience: Be a registered paramedic, with current HCPC registration and have post-registration experience, ideally with some being in primary care. MSc in Advanced Clinical Practice (or equivalent experience. Independent prescriber (V300 qualification). Experience in primary care, community nursing, or urgent care settings. Strong clinical assessment, diagnostic, and prescribing skill. Full UK driving licence and access to a vehicle for home visits.
