North Unplanned Social Care need a Band 6/level 2 Social Worker.
37.5 hrs (Full Time Permanent)
An exciting opportunity has become available in the North Unplanned care service for a permanent Band 6 Social worker. We need a resilient and dynamic individual to join the experienced and enthusiastic team.
The work is varied and challenging and will focus on pathway 2 of the D2A service. You will be supporting and assessing people within a rehab focused bed based setting following an acute hospital stay. The core work will be undertaking statutory assessments in complex situations so knowledge of Mental Capacity and Continuing Health Care legislation is crucial. A knowledge of safeguarding adults will also be an advantage.
The successful applicant will be part of a multi-disciplinary team including nurses, physiotherapists, occupational therapists, discharge facilitators and support staff within the Haywood hospital and private residential and nursing homes. The team also works closely with a number of external partners such as the ICB and Staffordshire County Council. The work is on a short term basis and cases are usually closed within 12 weeks this means the successful individual will need to be able to work at pace and be able to move between competing priorities.
Main duties of the job
This is an ideal opportunity for any individual that wants to work within an unplanned care setting and access a variety of training via the social work learning academy (SWLA) where you will also have an annual my time meeting to consider your professional development and identify career goals.
The notional base for this post is the Haywood Hospital but flexible/blended working is supported and following a period of induction staff will work mostly from home to complete documentation, with regular face to face visits within the D2A settings. Daily support is offered via a team group chat on Microsoft teams and an open door policy from management.
Going forward this post may involve some weekend working on a rota basis and also offers consideration for 9 day working.

Social Work Learning Academy (SWLA)
On appointment, the post holder will be auto-enrolled in the SWLA and aligned to the appropriate British Association of Social Work Professional Capabilities Framework. The relevant SWLA stage (i.e. stage 1 (ASYE), 2 (Social Worker) or 3 (Advanced Social Worker) will also be agreed taking all prior Continuous Professional Development (CPD) and relevant experience into consideration. The post holder will be required to take responsibility for their own Continuous Professional Development.

Essential
* Professional Social Work qualification, DipSW, CQSW, CSS, BA, MA or Foreign qualification where certified by the Professional Social Work regulatory body
* Ensuring current registration with the Professional Social Work regulatory body at all times.
* To hold/to undertake and achieve the status of Approved Mental Health Professional and/or Best Interest Assessor as per the SWLA Career Pathway
* To undertake and achieve the status of Work Based Supervisor and Practice Educator as per the SWLA Career Pathway
* Good keyboard / IT skills including word processing, spreadsheets, email and PowerPoint
* An ongoing commitment to personal and professional development.
* A clear understanding of and commitment to equal opportunities and anti-discriminatory practice

* Existing accreditation relating to Practice Educator/Work Based Supervisor

* Significant post-qualification experience of working with adults with needs associated to older age, physical health or mental health needs and their carers
