Community Practitioner
Band 6
Hello!
We are so delighted you are considering coming to work with us at Cumbria, Northumberland, Tyne and Wear NHS Foundation Trust (CNTW).
We know the power of looking into an organisation, one you might want to work in, or one where you are going to get help, and being able to say, ‘I’ve seen somewhere I can fit in and be myself.’ We’re on a journey to make the Trust an inclusive employer, representative of the community we serve and we want more of our community to consider applying for jobs with us.
Job overview
The Children and Young People’s Service provides an integrated service for children with persistent, moderate to severe mental health concerns, who have received a previous targeted intervention from other services and have shown little or no improvement.
We deliver a wide range of evidence-based interventions through clearly defined care pathways. Development opportunities can be accessed via our comprehensive internal training programme. We have a robust supervision structure for staff, alongside peer support.
Our Mental Health pathway undertakes assessment and intervention to children and young people with complex mental health needs, including anxiety, low mood, self-harm, and trauma. Clinicians will work within a collaboratively care planned approach, should be able to work autonomously and within a multi-disciplinary team.
We are keen to recruit clinicians with experience in working with children and young people, and who can work collaboratively with partner agencies.
The team operates across the hours of 8.00 to 8.00 Monday to Friday, and Saturdays 9-5.
Main duties of the job
1. Carrying a complex caseload of young people requiring therapeutic interventions.
2. Provide training and clinical supervision/support to the wider team.
3. Attendance at multi-agency meetings and risk management group.
4. Support with transition work and audits as appropriate, linking in with parents, carers, and other partners.
