Overview
We are seeking a skilled Clinical Pharmacist to join our healthcare team. The ideal candidate will play a crucial role in providing pharmaceutical care to patients in a clinical setting.
Hours: 37.5
Salary: £46,000 to £60,000 depending on experience
Reporting to: The Clinical Pharmacist will report to the Principal GP for clinical matters and to the Senior Management Team for non-clinical matters.
Job Summary and Scope
The Clinical Pharmacist is responsible for implementing an effective medicine management system within the practice. This will include, but is not limited to, cost-effective prescribing, working as an independent prescriber, providing advice and support for both patients and colleagues. In addition, the post-holder will also support the multi-disciplinary team, in line with the strategic objectives of the practice.
Key Responsibilities – Clinical
Caring for patients
1. Practice as an independent prescriber working within scope of practice
2. Carry out face-to-face and telephone consultations with patients with minor illnesses
3. Provide medication review services with patients in the practice and during domiciliary visits to the local nursing home
4. Provide advice and support for patients with self-limiting conditions
5. Re-authorise medication for repeat prescribing within scope of practice
6. Reconcile medicines following patients discharge from hospital or clinic appointments
7. Reconcile medications following correspondence from third party providers e.g. secondary care
8. Review medications for newly registered patients
9. Implement and embed a robust repeat prescribing system
10. Carry out targeted medication reviews for high-risk patients
11. Identify patients suitable for repeat prescribing, liaising with the relevant clinicians as required
12. Review and implement safe prescribing systems for vulnerable patients, improving compliance
13. Support clinicians with the management of patients suffering from drug and alcohol dependencies
Communication with patients
1. Ensure patients and carers are aware of, and acknowledge, the Clinical Pharmacist role, including its scope and limits
2. Advise and process referrals for patients who need to be seen by a GP or other appropriate member of the clinical team
3. Signpost additional information and support for patients, including making referrals as appropriate.
4. Use communication that is appropriate to the situation, including communicating sensitively with patients and carers when delivering bad news
5. Establish appropriate communication methods to suit patients’ and carers’ level of understanding, cultural/language needs, and preferred communication method
6. Anticipate and overcome barriers to communication, and seek support from other staff members where necessary
7. Use appropriate communication skills and style to make patients aware of risk, to obtain consent where necessary, and to ensure patients comply with treatment
8. Advocate for patients who need additional support and guidance
Delivering high-quality services
1. Provide services to the practice’s patient population, to the highest possible standard, in line with competencies and professional code of conduct
2. Monitor and maintain the standard of care provided, taking appropriate action if standards are not met, through self- and peer-review, benchmarking and evaluation
3. Participate in clinical governance and clinical audit activities to ensure the continual improvement of service delivery
4. Where appropriate, participate in reviewing and responding to complaints, significant events and other shared learning activities within a structured framework
5. Support the performance of team members and collaborate in improving the quality of healthcare and service provision, in line with local and national policy and strategy
6. Work within the practice’s legal framework for identifying vulnerable patients and applying policies relating to the treatment of vulnerable patients
7. Encourage cost-effective prescribing within the practice
8. Review all safety alerts ensuring the required actions are taken
9. Provide subject matter expertise on medication monitoring, implementing and embedding a system
10. Assist with QOF targets
Key responsibilities – non-clinical
Management and leadership
1. Act as a positive role model, and set good practice examples and standards for the wider practice team.
2. Provide leadership and support to the Medicines Management Team within the Practice.
3. Participate in the development and implementation of clinical guidelines, in line with national and locally agreed protocols and up-to-date evidence base
Practice-related duties
1. Accurate record-keeping, including (but not limited to):
2. Making and maintaining accurate, contemporaneous records of encounters with patients
3. Ensuring data is recorded to enhance patient care (eg, coding, reminders) and in an accessible way, to facilitate data analysis
4. Provide data analysis for the Practice team as required
Participate in implementing the maintenance of a safe working environment, in accordance with health and safety protocol, including (but not limited to):
1. Keeping clinical and non-clinical working areas tidy and free of hazards, minimising the risk to everyone in the practice
2. Adhering to policies relating to infection control, cold chain, spillages, decontamination, handling of specimens, handling sharps and clinical waste, etc
3. Maintaining up-to-date knowledge on health and safety protocol
4. Leading, contributing to and attending mandatory training when required
Secondary Responsibilities
In addition to the primary responsibilities, the clinical pharmacist may be requested to:
1. Administer influenza and travel vaccines
2. Provide education on medicines for the clinical team
3. Review the latest guidance ensuring the practice conforms to NICE, CQC etc.
4. Act as the practice point of contact for all medicines related matters, establishing positive working relationships
5. Work and liaise with Clinical Pharmacists and other Allied Health Professionals as needed to provide appropriate patient care
Other important aspects of the role
Maintaining confidentiality
The Clinical Pharmacist will, as an integral part of and in the course of carrying out his/her role, have access to confidential and sometimes sensitive information relating to patients, carers and family members, as well as similar information about colleagues. The Clinical Pharmacist might also have access to confidential commercial information about the practice and its business.
All such information, in whatever format and howsoever made available, must be treated with the strictest confidence.

QUALIFICATIONS
ESSENTIAL
1. GPhC Registered Pharmacist
2. GPhC Independent Prescriber Qualification
3. Minor Ailments certification
DESIRABLE
1. Medicines Management Qualification
2. MUR (medicines use review) and repeat dispensing certification
EXPERIENCE
ESSENTIAL
1. Minimum of 2 years post registration experience
2. Minimum of one year working as a Clinical Pharmacist in General Practice
3. Evidence of appropriate continuing professional development activity to maintain up-to-date knowledge and on-going competence in all aspects of the ANP role
4. Experience in managing pharmacy services in primary care
5. Evidence of working autonomously and as part of a team
6. Proven ability to evaluate the safety and effectiveness of their own clinical practice
DESIRABLE
1. Interpreting and implementing local and National policy agendas for health
2. Knowledge of the GPMS/APMS contract
KNOWLEDGE :
ESSENTIAL
1. Understanding of systems to gain an understanding of the health needs of the Practice population as they relate to primary care
2. Understanding of evidence based practice
3. Knowledge of national standards that inform practice (e.g. National Service Frameworks, NICE guideline, SIGN guideline, etc)

5. Experience of use of a medical software package (ideally SystmOne)
