Job summary An exciting opportunity has arisen to become a part of the cancer team within the Gynaecology department as a Patient Pathway Co-Ordinator. This post is pivotal to ensure timely patient pathway progression for both before and after cancer diagnosis. We use the Somerset Cancer Register information system to collect real-time data throughout the patient's referral, diagnostic and treatment pathways. The data aids the clinical management of the patients and informs progress against the national Cancer Waiting Times standards. The main objectives of the role are: Working with members of the clinical and admin teams to track patients through their cancer pathway. Pro-actively monitor individual cancer patients to ensure that they progress through the pathway within the standard timelines and to prevent breaches of national Cancer Waiting Times standards. Collect and validate clinical data for mandatory cancer datasets. Provide appropriate cover support across the team during periods of annual leave and absence. You will also be required to offer support to our RTT validator. Monday - Friday - Hours to be discussed at interview, possible home working can be included. Main duties of the job To co-ordinate the patient journey from referral, through diagnostics, treatment planning and treatment completion, highlighting any potential delays in the pathway and ensuring accurate data collection to support the process in accordance with Departmental, Trust and Network guidelines. To work closely with members of the Cancer team and with other PPCs across the Trust in developing policies and procedures that ensure seamless management of cancer patients in all specialties. To maintain up to date knowledge of all targets and processes relating to Cancer management and to be a point of reference within the department for other members of the Multi-Disciplinary Team. This role will require the post holder to work closely with Clinicians, Specialist Nurses, Secretaries and all members of the Multi-Disciplinary Team.
