The Community Wellbeing Team works closely with the Swansea Bay University Health Board to provide a Reablement service as well as short- and long-term care to individuals living within the Neath Port Talbot County Borough. To apply for this role you must be able to work as an effective team member helping to deliver short & long term programmes of care or therapy to individuals who have become unwell or lost their skills and confidence for living independently. You will provide a high standard of personal care and support to people within their own home, taking direction from supervisors & therapists to implement individuals support plans. Previous experience of working with individuals in need of care or support in a health and social care setting is desirable, along with an understanding of supporting people living with dementia, however most importantly we want you to have a passion for supporting our individuals who receive this care & support. We can provide you with training and will help you work towards a Level 2 qualification in Care. If you already have this qualification, we would love to hear from you. We will also help you register as a Domiciliary Care Worker with Social Care Wales which is essential for this role. You need to be caring, enthusiastic, empathetic and able to travel extensively within the County Borough area.
